A US healthcare provider needed a single platform to unify appointment booking, client referrals, group scheduling and hospital callbacks across multiple clinics. Built with React and Node.js, delivered in 12 weeks.
The Brief
A US healthcare provider operating across multiple clinics had three separate legacy systems for appointments, referrals and callbacks. Clinical staff were switching between platforms constantly, leading to scheduling conflicts, missed referrals and poor patient experience.
They needed a single unified platform accessible from any device - where patients could book appointments, clinicians could manage referrals, and administrators could oversee group scheduling and hospital callbacks, all in one place.
"We have three systems that don't talk to each other. Staff waste 30 minutes every morning just reconciling calendars. We need one platform that does everything and patients need to be able to book themselves."
What Made This Hard
Healthcare platforms require HIPAA-compliant data handling - every design decision had to consider patient privacy, data residency and access controls. Role-based permissions had to be granular: patients see their own data only; clinicians see their assigned patients; admins see everything.
The group scheduling feature was particularly complex sessions could have multiple clinicians, multiple patients, recurring schedules, and needed to handle cancellations and rescheduling without breaking references. The hospital callback queue had to be real-time, with clinicians notified the moment a request came in.
Project Phases
Mapped all four workflows individual bookings, referral management, group scheduling, and callback queues into a unified data model. Designed a PostgreSQL schema that handled all relationships. Chose React for the frontend and Node.js/Express for the API, deployed on HIPAA-compliant AWS infrastructure.
Built JWT-based authentication with three distinct roles: Patient, Clinician and Admin. Each role sees a completely different UI patients see their appointment history and upcoming bookings; clinicians see their schedule and assigned referrals; admins have full system visibility. Implemented session expiry and secure password reset via email.
Built the core booking engine patients select a clinic, service type, clinician and available slot. Availability is computed in real time from the clinician's calendar, accounting for existing appointments, blocked times and clinic operating hours. Integrated Twilio SMS for automated appointment reminders sent 24 hours and 2 hours before the appointment.
Built the referral module clinicians can refer patients to specialists, with tracking of referral status (sent, accepted, completed). Group scheduling supports recurring sessions with multiple participants, full conflict detection and cascade rescheduling. Both features had extensive edge-case handling for cancellations, no-shows and participant changes.
Built a real-time hospital callback queue using WebSockets requests appear instantly in the clinician dashboard. Implemented priority levels, automatic escalation after 30 minutes and audit logging for all callbacks. Load tested to 1,000 concurrent users before launch. Deployed with zero downtime and handed over to the client's team.

The platform launched on schedule and was adopted quickly by clinical staff the unified workflow eliminated the daily reconciliation burden. Patients responded strongly to the self-service booking, reducing front-desk phone volume significantly.
